External Professional Activities Request Form

在 review of an External Professional Activities Request Form, the reviewer(s) will determine if the requested activity is compliant with USA policies and procedures, and then communicate their determination to the 请求者. Submitting an External Professional Activities Request Form

1. 去 COI风险经理 and log-in just as you would for submitting your annual Disclosure
2. Sign in with your JAG number and JagNet Password.
3. On your 首页 page in the section labeled, "Request forms available to submit," click the blue "Fill Out" button alongside the "环保局要求 Form" label, to begin.  在 completion, submit the form. It will be routed to  your supervisor for their review. The Dean will also review, and either approve or deny the request.
